Primary chronic osteomyelitis of mandible is a rare disease whose treatment and onset is still in debate. It mainly occurs in the child but it is not age-specific. Further, immunocompromised states like juvenile diabetes mellitus make children more prone to osteomyelitis as it lower down their immunity. Chronic osteomyelitis must be diagnosed at an early stage to avoid morbid state and its long-term complications. Management strategies should be focussed to control underlying systemic disease and thorough surgical debridement and antibiotic administration based on culture and sensitivity.
